Yes i googled 'Retractable Toothpick' straight after i posted that reply and got loads of stuff so there we are!

Just a wild stab in the dark because i know nothing about antiques, but it could be a smelling pot. it might have had some pleasant smelling herbs in esp. if the original owner worked in impoverished areas!
