It's not necessarily for traveling, and it's not specifically for the last sacrament.
You're very close; I can see this post will soon need its last sacrament.

All those years at Our Lady of Fatima finally paid off! I think someone actually posted a question about one here recently, think it has gone unanswered, but can't seem to track it down.
